Below you will find every thing you need to register for the camp. Please print and fill out the Medical History Questionnaire, Waiver and Release of Liability and the Registration Form and send it to the address below with a check written out to “Minnesota cross country camp”. We will also need a photo copy of your insurance card. Registration must be postmarked by June 23, 2008. Thanks for your interest in the Minnesota Cross Country Camp and hope to see you in July!
